For the treatment to be performed, the Client's consent is required, for which the Client signs the treatment agreement before treatment.
Treatments with injectables performed by Doctors at Soap are prohibited for minors in the Netherlands. This means that a Client must be at least 18 years old. Only with a referral from the GP or medical specialist will Doctors at SOAP perform a treatment under the age of 18 for medical necessity.
Treatments by the skin therapist can take place at any age; for clients under 16, written consent from a parent or legal representative is required, and for clients aged 16 and 17, the clinic may require additional consent, depending on the nature of the treatment.
3. Care

If necessary, Doctors at Soap makes notes in the digital file regarding the health of the Client and the procedures performed.
At the Client's request, a copy of the medical file can be requested from Doctors at Soap. This must be done personally. For this, the client must provide identification.
At the Client's request, Doctors at Soap can add a statement issued by the Client regarding the documents included in the file to the file.
Doctors at SOAP deletes a file at the Client's request. The Client receives a request for destruction. This must be filled in and sent. Upon receipt of this form, the request will be assessed. If there are no objections, the Client receives a confirmation with the request to sign and return it. A maximum of 3 months after receipt of this confirmation, the requested data will be destroyed. The request for destruction is contested. Destruction is omitted if it is reasonably plausible that retention is of significant importance to the Client and if destruction is not allowed according to the law.
